Please bring this referral form with you to your first appointment. If you are an approved patient under the CDM plan, you will be entitled to a Medicare rebate for physiotherapy services, once you have paid your invoice in full, at the time of consultation.All of our clinics have the HICAPS machine available, so we can process your claims on the spot!
